CRESTVIEW, Fla. (WDNews) — Crestview police are asking for the public’s help locating a 15-year-old who has been missing since June.

According to the Crestview Police Department, Jacob Carlisle Cook was reported missing by his caregivers June 15 after he failed to return from an authorized walk around 3:45 p.m.

Police said another juvenile who was with Cook told investigators the two planned to visit Walmart and GameStop after stopping at Publix on South Ferdon Boulevard. When the other juvenile came out of Publix to meet Cook outside, Cook was reportedly gone.

“Thus far, our investigation has led us to believe that COOK is still in the Florida panhandle and is with subjects known to him,” the department said.

Police said Cook is not believed to have a cellphone, and investigators do not know his exact destination or location.

The department issued an updated bulletin after officials said multiple posts about the case were circulating on third-party social media pages, some containing inaccurate photographs of Cook.

Cook is described as 5 feet tall and weighing about 100 pounds. He has short red hair and brown eyes. Police said he has a scar on his left eyebrow and cutting marks on his right arm.

He was last seen wearing a gray hoodie, gray sweatpants, a white T-shirt and carrying a backpack.

The Crestview Police Department said the case remains active.

Anyone with information about Cook’s whereabouts is asked to call the Crestview Police Department at 850-682-2055 or contact Emerald Coast Crime Stoppers to remain anonymous.
